Does this account for collars?

Yes. You can include collars so the effective bar weight reflects how you actually load the setup.

What if my gym does not have every plate size?

Disable the missing plate sizes and the calculator will give the best available loadout from the plates you selected.

Why is there sometimes a remainder?

A remainder means your target cannot be matched exactly with the enabled plates, so the tool shows the closest valid setup below it.
